Complete this form for each and every outing a Scout attends. This helps us to contact you in case of emergency.
Fill out the Class 1 medical form (first page) annually, and have your health-care provider fill out the Class 2 medical form (second page) every three years.
If you are an adult over 40, have your physician fill out this Class 3 medical form annually. Also, youth participating in high adventure activities need to have this form filled out annually.
Use this form to figure the amount you owe for your annual registration fees. This form needs to be filled out and submitted to the Troop Treasurer every November.
